The front of the card has room for entering personal information and states i.e., Believing in the principals and objects of your order I hereby respectfully apply for admission in your Tribe. In consideration of admission thereto, I agree that if elected to membership I will obey the laws and usages of the Order as they now exist or may be hereafter enacted. I am a white citizen of the United States, have been a resident of this reservation for the last six months; I speak the English language; I believe in a Supreme Being; am sound in mind and body; have not been rejected in any tribe within six months and have a reputable means of support. And the back of the card has the Official Seal and signature of the Great Chief of Records. The wording around the seal says Great Council Improved Order of Red Men Inst. 19th Sleep Worm Moon G.S.D. 356.
